Certified asbestos consultant/site surveillance technician year 2009 exam schedule

The Asbestos Consultant and Site Surveillance Technician Certification Examinations are given at locations in both southern and northern California.  A completed application (including all required documentation) should be submitted at least three (3) weeks before a date shown below to assure adequate time for review of the application and for correction of any deficiencies. Although we will review applications whenever we receive them, there can be no assurance of admission to that particular exam when any of the required materials is missing or received later than the submittal due date, whether by fax or any other means.  Your admission to the exam will be confirmed by an invitation letter; please bring the letter with you to the exam location.  Complete the lower portion of this form and submit it along with your application materials.

Re-exam: If you did not pass the exam the first time, you are entitled to retake it one time on the same application.  To retake the exam, complete the lower portion of this form, and submit it along with a check or money order for 50% of the application fee.  It is not necessary to submit a new application.
