Register or Renew by Mail:

1. Download and print the application.
2. Complete and sign the application.
3. Mail your application with the following items:

   ✓	Copies of all required documents.
   ✓	A cashier’s check or money order for $500, made payable to DLSE. Personal checks or business checks are not accepted.
   ✓	Declaration and Authorization to Release Information (optional)
