Approved OCR forms vendor list
The Electronic Adjudication Management System (EAMS) requires the use of optical character recognition (OCR) forms, which replaced many of the old "legacy" paper forms found at DWC district offices. These forms can be found and downloaded at http://www.dir.ca.gov/dwc/forms.html#EAMSForms.
Private vendors have developed their own OCR forms that are available for public use. To make sure these forms are compatible with EAMS, the Division of Workers' Compensation (DWC) has done extensive testing of vendor-created OCR forms. The list below comprises those vendors whose forms passed testing.
The division does not endorse or guarantee the product(s) developed by the vendors listed below. Each vendor's software has been tested only for minimum functionality and compatibility with EAMS. Each software product is different and we recommend that you contact each vendor and select a product that best suits your individual needs.
