• When registering or renewing by mail, include the following:
• A completed printed application.
• Copies of all the required documents.
• Fees may be paid by certified check, cashier's check, or money order made payable to DIVISION OF LABOR STANDARDS ENFORCEMENT.
• Personal checks or business checks are not accepted.
• Declaration and Authorization to Release Information (optional)
