DLSE fill-in forms instructions

Most DLSE forms have been prepared to allow you to fill in the blanks with your personal computer.  You will need Acrobat Reader or Acrobat 4.0 or greater to enter data, view, and print the forms.

Acrobat Reader allows you to view the PDF, enter data and print only.  Acrobat Reader is a free download from Adobe at: http://www.adobe.com/products/acrobat/readstep.html.

Download the form that you want to use.  Load it into Acrobat Reader.  Your cursor will blink in the first field.  Just key your data into the field.  You can tab to the next field until the form is completed.  If you are using Acrobat Reader, you cannot save your data, so you will need to print a copy for your records and a copy to mail to DLSE.  If you have Acrobat Exchange, you can save the data on the form for future use.
