Register or Renew by Mail
Steps to register by mail or in person:
- Download and print the application, available in multiple languages
- Complete and sign the application
- Mail your application to the Labor Commissioner’s Office nearest you with proof of your workers’ compensation coverage - one of the following:
- A valid workers’ compensation insurance certificate, which must include the complete and correct name of the legal entity that is the insured employer, including fictitious business names and the complete and correct address for each location
- Certificate of authority to self-insure
Important: The Labor Commissioner will not accept copies of an application for Insurance, policy declaration, information page, annual rating endorsement or payroll report. Only the actual certificate or a legible copy will be accepted.
