§ 13867. Amended Disclosure.



(a) If any of the information disclosed pursuant to sections 13865(c)(1) through (c)(3), inclusive, changes following the submission of an initial disclosure, the employing person shall complete, sign and submit an amended disclosure (DLSE 704 A) to the Labor Commissioner by email at foreignlaborcon@dir.ca.gov or by mail to: California Labor Commissioner, Licensing & Registration-FnLC, 1515 Clay Street, Suite 401, Oakland, CA 94612, within fifteen (15) days of the change.
(b) The amended disclosure (DLSE 704 A) shall identify the name of the employer (legal and “doing business as” names, if different), the date on the initial or previous disclosure submitted, indicate what information provided pursuant to section 13865(c) of this subchapter has changed, and shall provide updated information for each identified category of information that has changed.
