§12025.1. Forms for Notice to Answer.


Notice to Answer as set forth in Section 12025 above, shall be in the following form:

TO THE ABOVE NAMED                                , respondent.
YOU ARE DIRECTED to file at the office of the State Labor Commissioner,  (insert address)                     ,    a written pleading in response to the Petition to Determine Controversy within 20 days after the service on you of this notice. You are notified that unless you so file a written responsive pleading, the petitioner may apply to the Labor Commissioner for any relief demanded in the petition.
