Cal/OSHA Cites Tutor Saliba Koch Tidewater Joint Venture
after Investigation of Fatal Incident
SAN FRANCISCO - The California Division of Occupational Safety and Health (Cal/OSHA) yesterday, issued a citation to Tutor Saliba Koch Tidewater Joint Venture for a violation of state workplace safety regulations related to a fatal accident at the Richmond-San Rafael Bridge on July 30, 2004.
"As a result of our investigation inspectors determined that proper training was not provided which could have prevented the accident," Cal/OSHA Acting Chief Len Welsh said. "An accident-related serious citation was issued, meaning that the violation resulted in the death of the employee."
This serious citation carries a penalty of $18,000. Specifically, Cal/OSHA alleges that Tutor Saliba Koch Tidewater Joint Venture had an employee operate an Ingersol Rand winch without prior training. Failing to train an employee given a new job assignment violates state safety regulation 8CCR 3203(a)(7)(C). California law provides that a company may appeal Cal/OSHA citations and penalties within 15 working days to the Occupational Safety and Health Appeals Board in Sacramento.
