Changes to spinal surgery second opinion physician regulations go into effect December 15, 2004

Final Spinal Surgery Second Opinion Physician regulations have been approved by the state Office of Administrative Law and took effect on December 15, 2004. The emergency regulations that have been in effect since July 2, 2004, continue in effect except as changed by the final regulations. In addition to the changes that were proposed for the regulations, there is also a minor change in the application form (DWC Form 232).
