DWC to stop accepting some pre-August 2010 OCR forms
The Division of Workers’ Compensation will stop accepting outdated versions of some OCR forms beginning April 1.
Last year, DWC made changes to several of its forms that filers use in the Electronic Adjudication Management System (EAMS). The updated forms were posted on the website Aug. 2, 2010.
The changes included:
- Adding a box for lien conferences on the declaration of readiness to proceed (DWC-CA form 10250.1)
- Removing a box on the document cover sheet (form 10232.1) for the vocational rehabilitation unit (VOC), because the vocational rehabilitation program ended Dec. 31, 2009
- Amending the listing of document titles and document types on the document separator sheet (form 10232.2), both in the body of the sheet and on the PDF version. (For a complete list of the added and deleted titles, see below.) Also, VOC was removed from the list of selectable product delivery units.
While DWC directed filers to use the updated forms at the time of release, the division continued to accept the older versions during the transition. But starting April 1, the outdated forms will be rejected.
Filers who are filling out forms directly from the DWC Web site or e-forms filers logging into EAMS to fill out e-forms will not be affected. This is an issue only for filers who downloaded pre-Aug. 2 forms to their computers and have been using them to file new cases.
DWC worked with OCR forms vendors to ensure their forms were compatible with the changes. Clients can check with their vendor to make sure they are using the updated versions.
