Division of Workers' Compensation Medical Unit has new address

 The California Division of Workers' Compensation (DWC) Medical Unit has a new address. All Medical Unit correspondence, including qualified medical evaluator (QME) panel requests, utilization review plans and complaints, medical provider network (MPN) applications and material modifications, and other forms should be sent to:

The Medical Unit phone numbers have not changed. The DWC is currently working on QME regulations, which include updating all the QME forms with the correct address. Additionally, other forms, such as the spinal surgery second opinion and medical provider network forms will be updated through the regulatory process. In the interim, forms sent to the San Francisco post office box will still be delivered to the Medical Unit in Oakland.
