14300.42 - Requests from the Bureau of Labor Statistics for Data

The Bureau of Labor Statistics (BLS), or their designee, may send the employer:

• a letter stating that injury and illness information will be collected for the following  year, and
• an occupational injury and illness survey form

Survey form must be filled out for the year covered by the survey and promptly submitted to BLS (or their designee ) even if the employer is:

• exempt from keeping injury and illness records, or
• located in a "state-plan" state
