Citations were issued by the DOSH Santa Ana district office upon completion of an investigation after receiving a report that on March 22, 2012 a low pressure 2-inch natural gas line had been struck by a back hoe operator while excavating. The incident occurred at the Metropolitan Water District, Diemer Water Treatment Plant in Yorba Linda, CA While the investigation was ongoing, on April 25, 2012 a second back hoe operator struck the same 2-inch natural gas line in the same excavation while under the supervision of the same foreman. There were no reported injuries.

The Division issued citations including two citations classified as serious willful. KB Trim, Inc. had previously been inspected during an enforcement action on Feb. 16, 2011. At that time two Orders Prohibiting Use (OPUs) had been issued for imminent hazards posed by lack of proper guarding on various machines and equipment, including cloth cutting, cloth rolling, bench grinder, sewing machines and an electrical panel. On a revisit to the business on April 11 to verify abatement in order to remove the OPUs, employees were observed working on some of the OPU-tagged equipment.

The Division issued citations after concluding an investigation of a confined space fatality and two serious injuries. On January 21, 2011, an employee entered a vessel measuring approximately 7 by 7 feet with an opening of 18 inches in diameter. The vessel contained 237 liters of a mixture of 40% ethanol, buffers and human plasma. Air in the vessel had been displaced by Nitrogen gas which had been bubbled through the product during the 20 hours prior to the accident. The investigation indicates that the victim entered the vessel to measure the temperature of the mixture using a short probe temperature meter. This was the fifth time he had entered the vessel on his shift. He collapsed upon entering the oxygen deficient vessel atmosphere and was discovered by his supervisor. The supervisor instructed two other technicians to rescue the victim, and also collapsed upon entering. They were all extricated by the Los Angeles Fire Department. The first victim died on scene, and the rescuers suffered serious injuries. One of them remains hospitalized since the January incident while the second technician who attempted the rescue is in good condition.

The Division issued citations after concluding an investigation into an attack by an individual on an employee which resulted in serious injury. On December 11, 2010 the employee was assaulted by an inmate while he was escorting him off the ward. The employee was found wandering the grounds by co-workers and was taken to the hospital and treated for severe head injuries. The serious citations issued were for failure to correct the unsafe work practice of employees escorting individuals with poor violent impulse control without proper protective methods and failure to have an emergency alarm system to provide proper, effective and timely warning to police an emergency responders when an employee is experiencing an assault by an individual.

On October 24, 2010, she had apparently been asphyxiated, and a patient at the hospital has been arrested in her death. Factors believed to be involved in the fatality include lack of adequate employee alarm systems on the unit, non-existent alarm systems outside the units, inadequate police presence in the event of assaults, and no enforcement of written policies and procedures by the employer.

Citations issued in this fatality investigation. An employee fell approximately 15 feet, was engulfed in sandy soil and asphyxiated. He had been standing on pieces of wood lagging he was using as a work
platform, placed on the fork attachment of a Bobcat Loader. The employee was attempting to drive an 8" structural "I" beam into a 16 foot deep by 2 foot diameter hole inside a 5 foot deep excavation, using
a Mikasa portable vibrating tamping rammer when the accident occurred. The structural steel was intended to be part of a shoring system for the excavation. The tamping rammer dislodged from the fork
attachment and the wood the employee was standing on. The employee's supervisor had devised the pile driving procedure used at the time of the accident.

The excavation was not protected from cave-ins by an adequate protective system designed in accordance with Title 8 Construction Safety Orders. Cal/OSHA inspector returned the next day and observed two employees working in this excavation. A total of ten citations were issued including a $70,000 willful serious citation for employee not protected from cave-ins by adequate protective system.

20 feet from a press box / coaches' booth to stadium seats at Qualcomm Stadium on Oct. 19, 2009. Employee was descending the stairs to the press box when he bent over and fell, tumbling from the window
opening. A special order was issued requiring that the employer provide protection of window openings greater than 30" vertically and 18" horizontally by the installation of guardrails or other barriers.

This inspection was initiated by a complaint. From 2005 to 2007 there were four instances where employees wearing latex gloves manually dipped optical lenses in hydrofluoric acid to strip antireflective
and/or hard coats from lenses suffered burns due to tears in the gloves. Gloves worn by employees were subject to cuts from the sharp edges of lenses allowing acid penetration to the skin. One employee
suffered a partial amputation to his right thumb because of hydrofluoric acid induced damage to the bone.
