To obtain a verification, you must have at least one of the following:
- Farm labor contractor license number, or
- Farm labor contractor's name
In addition, please provide as much of the following information as is available to you:
- Doing business as (Dba)
- Corporate officer name(s)
- Partner name(s)
- Contractor address
You must provide your contact information:

- Your phone number (if we require additional information to complete the verification)
