Citations were issued to Disney Construction Inc. following an accident on May 30, 2014 that killed two workers. The company was building a concrete bridge between the Yolo and Solano Counties over Putah Creek. The employees fell 80 feet from a crane-hoisted platform in the City of Winters. Investigation revealed that Disney Construction’s crane had not been certified or visually inspected for defects, the crane operator failed to perform an unmanned trial run or have a competent person inspect the rigging before usage, and there was no qualified signal person to assist with the lifting operation.

Description: The employer did not provide exit of directional signs along the length of the consolidation facility at intervals necessary to provide the employees with knowledge of the six emergency exits available. Did not ensure that employees loading product from hand carts inside trailers were provided with clear walkways of not less than 24 inches in width. Two instances of not providing permanent walkways and aisles that were clearly defined by painted lines, or other methods of marking. Did not ensure that the employees who operated and/or work in close proximity to forklifts and electric pallet jacks in the facility were provided with appropriate foot protection. Had not immediately repaired or prevented working doors with broken dock plates. The employer failed to ensure that only qualified electrical workers were allowed to perform work or take any conducting object within an area where there is a hazard of contact with energized conductors. The employer's program was not effectively implemented with respect to the training provisions, in that the employer allowed employees, who had been given a new job assignment, to perform that job while having not completed the training. The employer did not develop and institute controls to safeguard personnel during railcar movement, and allowed workers to conduct work on the railway tracks where trains were travelling in excess of 65 miles-per-hour. Furthermore, the employer's control method, namely the "Simple Approval" procedure, does not safeguard personnel working on tracks during railcar movement.

Employees were not trained and instructed in the hazards involved in their job assignments. A job briefing was not conducted by a qualified tree worker before beginning the job assignment, and when significant changes were made that affected the employees safety. A qualified tree worker did not determine the appropriate rigging system for the removal of the tree at the work site. Wedges, block and tackle, and other lowering devices not used when there was a danger that the tree being removed could fall in the wrong direction.

The German multinational company has a manufacturing plant in Bay Point, where the accident occurred. The willful violations were issued for Henkel's failure to properly set up and maintain operation guards for the mixer involved; and for the failure to follow its own Injury and Illness Prevention Program provisions regarding the identification and correction of serious hazards posed by the mixer. The investigation also revealed the company continued to operate the mixer despite knowing that the partial guard on the mixer exposed workers to dangerous moving parts. One of the serious violations issued against Henkel was for the company's failure to ensure that sleeves on employee coveralls fit tightly so clothing would not get caught in machinery.

On August 6, 2012, a fire broke out at the Chevron Richmond refinery when a corroded pipe in Chevron's #4 Crude Unit began leaking. Cal/OSHA immediately launched an investigation into the fire and the leak repair procedures throughout the refinery, and found that Chevron did not follow the recommendations of its own inspectors and metallurgical scientists to replace the corroded pipe that ultimately ruptured and caused the fire. They also found that Chevron did not follow its own emergency shutdown procedures when the leak was identified, and did not protect its employees and employees of Brand Scaffolding who were working at the leak site. There were also violations in Chevron's overall implementation of its own "process safety management" (PSM) procedures, required by Cal/OSHA of all refineries.
